Электрофизиологические аспекты кардиотропного действия нового отечественного антиаритмического препарата РГ-2 (реферат, курсовая, диплом, контрольная)
Актуальность исследования.
Фибрилляция предсердий (ФП) является самым распространенным нарушением ритма сердца, которое обнаруживают у 0,5−1% всех жителей развитых стран (Savelieva et al., 2000). С возрастом частота встречаемости ФП значительно увеличивается, достигая 9% у лиц в возрасте 80−89 лет (Kannel et al., 1998; Nattel et al., 2002). На фоне ФП повышается вероятность возникновения коронарных тромбоэмболий и инсультов, кроме того, увеличивается уровень смертности (Aronow, 2002; Mattle, 2003). ФП является очень частым (до 50%) осложнением после кардиохирургических операций, и ее предотвращение позволяет уменьшить время госпитализации и вероятность развития инсультов (Morady, 1999; Auer et al., 2004). Несмотря на то, что структурные изменения изменения миокарда встречаются в большинстве случаев ФП, она может развиваться и в нормальном сердце, в том числе у молодых пациентов (Prystowsky, 1996). В настоящее время установлено, что одной из причин возникновения ФП может служить повышенный тонус блуждающих нервов (Coumel, 1994; Hohnloser et al., 1994).
Долгое время основными средствами для лечения ФП являлись препараты I класса по классификации Vaughan-Williams (Vaughan Williams, 1984а). Однако во многих исследованиях, в том числе и многоцентровых, показано, что препараты I класса, обладая прямым антиаритмическим действием, в отдаленном периоде не только не снижают, но могут и увеличивать смертность больных (Greene et al., 1992). Кроме того, различные экспериментальные и клинические исследования свидетельствуют о более высокой антиаритмической эффективности препаратов III класса по сравнению с I (Rensma et al., 1988; Edvardsson, 1993; Camm et al., 1999; Nattel et al., 1999). В связи с этим, в настоящее время развитие фармакологических методов лечения ФП направлено на создание новых антиаритмических препаратов III класса (Nair et al., 1997; Singh, 2004).
Известно, что поддержание ФП определяется механизмом циркуляции возбуждения (reentry) (Мое et al., 1964; Allessie et al., 1985). В экспериментальных условиях стимуляция блуждающего нерва вызывает неоднородное укорочение предсердного рефрактерного периода и длины волны (ДВ) возбуждения, создавая, таким образом, условия для возникновения и поддержания циркуляции возбуждения (Allessie et al., 1985; Rensma et al., 1988). В экспериментальной модели ваготонической ФП антиаритмики III класса, такие как соталол, амбазилид, MS-551, нибентаи, показали высокую антиаритмическую эффективность (Wang et al., 1994аHayashi et al., 1998; Fedorov et al., 2000), которая объяснялась, главным образом, значительным увеличением ДВ возбуждения за счет прироста рефрактерности.
Некоторые антиаритмики III класса, такие, как соталол, MS-551, амбазилид и нибентан обладают холинолитическим эффектом, который, по-видимому, обуславливает более высокую антиаритмическую эффективность препаратов III класса, обладающих холинолитическим действием при купировании холинергической ФП (Wang et al., 1994аHayashi et al., 1998; Fedorov et al., 2000).
РГ-2 — новый отечественный антиаритмический препарат III класса (ВНИХФИ, Р. Г. Глушков с сотруд.), который по предварительным данным, полученным Южаковым С. Д. с соавт., показал на модели электрически вызванной фибрилляции желудочков у кошек высокую антиаритмическую активность, а также обладал низкой токсичностью. Однако электрофизиологические механизмы антиаритмического действия РГ-2 до конца не выяснены. Цель исследования.
Изучить механизм кардиотропного действия РГ-2 и его антиаритмическую эффективность на ваготонической модели ФП. Задачи исследования.
1. На изолированных кардиомиоцитах крысы исследовать влияние РГ-2 на потенциалуправляемые ионные токи.
2. С помощью микроэлектродной техники исследовать влияние РГ-2 на параметры ПД волокон рабочего миокарда предсердий крысы и кролика в норме и на фоне холинергической стимуляции карбахолом.
3. На модели ваготонической ФП у собак в условиях сердца in situ оценить антиаритмическую эффективность РГ-2 и изучить механизм его действия.
4. С помощью метода электрофизиологического картирования сердца собаки проанализировать причины прекращения и предотвращения холинергических ФП под действием РГ-2.
5. Провести исследование действия РГ-2 на электрофизиологические параметры сердца собаки in situ в зависимости от частоты сердечного ритма в норме и на фоне стимуляции блуждающего нерва.
6. Сравнить эффективность и механизм кардиотропного действия РГ-2 с первым отечественным антиаритмическим препаратом III класса нибентаном.
Научная новизна.
1. В работе впервые показано, что антиаритмический препарат III класса РГ-2 в диапазоне концентраций, селективно увеличивающих длительность ПД, обладает холинолитическим действием.
2. РГ-2 в диапазоне клинических доз прекращал ваготоническую ФП и предотвращал ее повторное возникновение. Картирование эпикардиалыюй поверхности предсердий показало, что под действием РГ-2 происходило увеличение размеров функциональных контуров циркуляции возбуждения, когда их размер достигал критической величины, возбуждение блокировалось, и ФП прекращалась. Эффективность РГ-2 в прекращении холинергической ФП связана со значительным увеличением рефрактерности в предсердиях во время быстрого предсердного ритма на фоне постоянной стимуляции блуждающих нервов, что делало невозможным существование циркуляции волны возбуждения по предсердиям.
3. Впервые показано, что действие РГ-2 на рефрактерность предсердий и желудочков не зависит от частоты ритма.
Теоретическая и практическая значимость.
Теоретическая значимость представленных результатов определяется существенным вкладом, который на примере РГ-2 внесен в понимание важности различных механизмов действия препаратов III класса для их антиаритмической эффективности при лечении ФП. Исследование показало, что холинолитическое действие может вносить существенный вклад в антиаритмическую эффективность РГ-2. Анализ хронотопографии возбуждения в момент окончания холинергической ФП в сердце собаки in situ может иметь практическую и теоретическую значимость для разработки новых подходов к лечению нейрогенных аритмий. Практическая значимость работы обусловлена экспериментальным исследованием электрофизиологических свойств нового антиаритмического препарата РГ-2, который может быть рекомендован для клинических испытаний. Кроме того, применяемые в данной работе модели ФП, имеющие функциональную природу, могут быть использованы для тестирования и исследования новых антиаритмических препаратов.
выводы
1. По спектру электрофизиологического действия (увеличение длительности потенциалов действия и отсутствие влияния на другие параметры потенциалов действия в опытах in vitro и увеличение предсердных и желудочковых рефрактерных периодов и интервала Q-T в опытах на сердце in situ) РГ-2 относится к антиаритмическим препаратам III класса по классификации Vaughan-Williams. Увеличение длительности потенциалов действия и рефрактерности под действием препарата обусловлено подавлением калиевого тока задержанного выпрямления 1к.
2. РГ-2 в диапазоне концентраций, увеличивающих длительность потенциалов действия, обладает холинолитическим действием. Таким образом, РГ-2 препятствует укорочению длительности потенциалов действия и рефрактерности в предсердиях, происходящему при повышенном тонусе парасимпатической нервной системы. Холинолитическая активность РГ-2, по-видимому, лежит в основе более выраженного изменения рефрактерности под действием препарата в предсердиях.
3. Действие РГ-2 на рефрактерность предсердий и желудочков наркотизированных собак не зависит от частоты ритма сердца.
4. РГ-2 в диапазоне доз 10−40 мкг/кг эффективно прекращал ваготоническую ФП и предотвращал ее повторное возникновение.
5. Антиаритмическим механизмом действия РГ-2 в данной модели является значительное увеличение рефрактерности во время быстрого предсердного ритма на фоне постоянной стимуляции блуждающих нервов, вследствие чего становится невозможным существование циркуляции волны возбуждения.
6. РГ-2 обладает похожим механизмом кардиотропного действия с первым отечественным антиаритмическим препаратом III класса нибентаном, но действует в концентрациях в 10 раз более низких, чем нибентан.
ЗАКЛЮЧЕНИЕ
Фибрилляция предсердий остается наиболее распространенным нарушением ритма сердца (Savelieva et al., 2000; Nattel et al., 2002). В последенее время все больший интерес в мире стали привлекать разработка и исследование антиаритмических препатов — представителей III класса по классификации Vaughan Williams (Sager, 2000). Новые препараты III класса показывают высокую антиаритмическую эффективность при лечении ФП, однако они не лишены и проаритмического действия (Nair et al., 1997; Gillis, 2000). Вместе с тем действие препаратов III класса на различные клеточные «мишени» (ионные каналы, насосы и рецепторы) до конца не изучено. Следовательно, механизм антиаритмического действия этих препаратов (in vivo) остается не совсем ясным.
Известно, что одной из причин возникновения ФП может служить повышенный тонус блуждающих нервов (Coumel, 1994; Hohnloser et al., 1994). Следовательно, холинолитическое действие антиаритмических препаратов, обнаруженное во многих исследованиях, может играть важную роль при лечении ФП (Zaza et al., 1995; Федоров et al., 1999a).
В настоящей работе показано, что новый отечественный антиаритмический препарат РГ-2 по спектру электрофизиологического действия относится к III классу по классификации Vaughan Williams. РГ-2 увеличивает длительность ПД и рефрактерных периодов, главным образом, за счет подавления калиевого тока задержанного выпрямления. Кроме того, РГ-2 обладает выраженным холинолитичеким действием, во время холинергической стимуляции он восстанавливает длительность ПД.
Изучение антиаритмического механизма действия РГ-2 в экспериментальных ваготонических моделях ФП показало, что его эффективность обусловлена значительным увеличением рефрактерности в предсердиях на фоне стимуляции блуждающего нерва, что делает невозможным существование циркуляции волны возбуждения по предсердиям. Кроме того, относительно избирательное действие РГ-2 на предсердия, обусловленное, по-видимому, холинолитическими свойствами препарата, может обеспечить низкую аритмогенность препарата. Таким образом, холинолитическая активность РГ-2 является важным компонентом высокой антиаритмической эффективности и безопасности препарата.
