Современный подход к медикаментозной терапии хронического цистита (реферат, курсовая, диплом, контрольная)
1. Актуальность проблемы.
Циститы являются наиболее частыми урологическими заболеваниями. Ежегодно около 3 миллионов пациентов обращаются к врачу по поводу различных форм цистита только в США (Foxman В., 2002 г.) [101].
Бактериальный цистит чаще относится к неосложненной инфекции нижних мочевых путей. Около 25% женщин в течение жизни переносят цистит в той или иной форме, из них 25−40% испытывают рецидив заболевания в ближайшие полгода (Naber K. G, 1998; Hooton Т.М., Stamm W. E 1997). Диагноз рецидивирующий (хронический) цистит подразумевает 2 обострения цистита в течение 6 месяцев или 3 обострения в течение года [19- 109] (Guidelines on urological infections. European Association of Urology, 2010), что оказывает значительное влияние не только на качество жизни пациенток, но и большое социально-экономическое воздействие на общество в целом. [99]. Лечение рецидивирующей инфекции нижних мочевых путей крайне непростая задача. Длительные курсы антимикробной терапии в субингибирующих дозах имеют положительный эффект лишь во время лечения [43−278], после прекращения терапии вновь отмечаются частые рецидивы инфекции. Более того, такая тактика ведет к развитию резистентности микроорганизмов к антимикробным препаратам [231]. Вакцинотерапия [64−135] (Уроваксом и др.) [94] пока мало оправдывает возлагаемые на нее надежды. Внутрипузырное применение гиалуроновой кислоты [70], является перспективным, но нуждается в проведении дальнейших рандомизированных исследований.
В то же время развитие инфекционно-воспалительного процесса — это всегда двусторонний процесс, т. е. взаимодействие макро и микроорганизмов [35−36]. Уропатогенная кишечная палочка способна моделировать иммунный ответ хозяина (Guidelines on urological infections, European Association of Urology, 2010) Известна иммуномодулирующая роль внутриклеточных возбудителей инфекций, передаваемых половым путем, создающих предпосылки для развития бактериального цистита у женщин (Лоран О.Б., Синякова Л. А., Косова И. В., 2008, Кудрявцев Ю. В., 2009).
В последнее время появились экспериментальные работы, подтверждающие персистенцию кишечной палочки в клетках уротелия, создание там внутриклеточных бактериальных сообществ [234]. В то же время и сама тканевая реакция слизистой мочевого пузыря может быть неадекватной, вследствие разных причин врожденного и приобретенного генеза, что также может вести к персистенции возбудителей и частым рецидивам заболевания [35−36−175].
Хронический цистит — заболевание, характеризующееся длительно существующей воспалительной реакцией в ткани мочевого пузыря, не только бактериальной природы. Воспалительная реакция должна характеризоваться не только временным показателем, но, прежде всего, характером тканевой реакции, что имеет наибольшее значение при выборе адекватной лечебной тактики (Ю.В. Кудрявцев, Т. С. Перепанова, П. Л. Хазан, 2003).
Известно, что хронический цистит может протекать в виде как экссудативной, так и продуктивной формы воспаления. При экссудативном воспалении закономерно наступает фаза пролиферации или репаративной регенерации (Левин Е.И., 1991). Вместе с тем, в тканях мочевого пузыря может развиваться несколько фокусов экссудативного воспаления через незначительные промежутки времени, т. е. происходит наслоение продуктивной фазы предсуществующего эпизода острого экссудативного воспаления на экссудативную фазу последующего (Ю.В. Кудрявцев, Т. С. Перепанова, П. Л. Хазан, 2007).
Таким образом, возникает необходимость дифференцировать ряд ситуаций, отражающих особенности тканевой реакции в стенке мочевого пузыря имеющих различное морфологическое проявление и, следовательно, требующих выбора различной лечебной тактики [О.Б. Лоран, Л. А. Синякова, И. В. Косова 2004; Ю. В. Кудрявцев, Т. С. Перепанова, 2003].
Все вышеперечисленное определяет актуальность темы, и указывает на необходимость ее досконального изучения.
2. Цель работы.
Улучшить результаты лечения больных с хроническим циститом на основе моделирования адекватной тканевой реакции слизистой мочевого пузыря стабилизаторами глюкозаминогликанового слоя.
3. Задачи работы.
1. Определить клинико-морфологические параллели разных форм хронического цистита.
2. Определить имуногистохимические и морфологические критерии разных форм воспалительной реакции в стенке мочевого пузыря при различных нозологических формах хронического цистита.
3. Обосновать в эксперименте возможность применения стабилизаторов глюкозаминогликанового слоя, для восполнения его дефицита при хроническом цистите.
4. Определить клинико-морфологическую эффективность и оценить безопасность применения стабилизаторов глюкозаминогликанового слоя при различных формах хронического цистита у женщин.
4. Научная новизна.
Обоснована необходимость биопсии стенки мочевого пузыря для дифференциальной диагностики различных форм хронического цистита у женщин.
Выявлены основные закономерности морфологических изменений в тканях мочевого пузыря при различных формах хронического цистита.
В эксперименте обосновано применение стабилизаторов глюкозаминогликанового слоя при различных формах хронического цистита.
Доказана высокая клиническая эффективность и безопасность применения гиалуроната цинка у женщин с различными формами хронического цистита.
5. Практическая ценность.
Разработан диференциально — диагностический алгоритм, который может быть использован при обследовании пациенток с симптомами нижних мочевых путей и синдромом хронической тазовой боли. Выявлены этиопатогенетические факторы при хроническом цистите и возможные механизмы рецидивирования. Проведено обоснование показаний и доказана безопасность применения гиалуроната цинка при бактериальном и интерстициальном цистите в эксперименте. Доказана эффективность и безопасность внутрипузырного применения стабилизаторов глюкозаминогликанового слоя при различных формах хронического цистита, сопровождающихся их дефицитом, что позволило значительно улучшить результаты лечения пациентов, повысить качество их жизни и увеличить безрецидивный период.
6. Основные положения, выносимые на защиту.
В отличие от результатов, полученных другими авторами, выявлено что: 1. Морфологические изменения стенки мочевого пузыря у больных с разными формами хронического цистита имеют характерные особенности.
2. При дезорганизации соединительной ткани стенки мочевого пузыря с дефицитом гликозаминогликанов показано их восполнение.
3. Внутрипузырное применение препарата гиалуроната цинка способствует улучшению состояния интерстициальной ткани и слизистой оболочки мочевого пузыря у больных с хроническим циститом.
4. Внутрипузырное применение стабилизаторов гликозаминогликанового слоя способствует улучшению результатов лечения больных с различными формами хронического цистита.
7. Внедрение в практику.
Результаты диссертации внедрены в клиническую практику в ФГУ НИИ Урологии Росмедтехнологий, а также в городской клинической урологической больнице № 47 г. Москвы, и в медицинском центре ООО «Прогресс-Мед», г. Москвы.
Выводы.
1. У пациенток с разными формами хронического цистита выявлены закономерности развития морфологических изменений в стенке мочевого пузыря:
• Агрессивные варианты воспалительного инфильтрата приводят к выраженным альтеративным изменениям в интерстиции и слизистой оболочке мочевого пузыря.
• Детерминация ассоциированной со слизистыми оболочками лимфоидной ткани (ACOJIT) системы по Th 1 типу приводит к развитию гранулематозных форм цистита.
• Детерминация ассоциированной со слизистыми оболочками лимфоидной ткани (ACOJIT) системы по Hi 2 типу приводит к развитию диффузно поверхностного цистита.
• При недостаточности факторов стимулирующих апоптоз (CD 95) развиваются атрофические варианты повреждения слизистой оболочки.
2. Определена степень дезорганизации интерстициальной ткани мочевого пузыря при различных формах хронического цистита.
3. В эксперименте доказана безопасность и высокая эффективность гиалуроната цинка, как препарата, способного стабилизировать систему гликозаминогликанов в интерстициалыюй ткани мочевого пузыря при разных формах хронического цистита.
4. Применение стабилизатора гликозаминогликанового слоягиалуроната цинка у больных с разными формами хронического цистита высоко эффективно (у 83,3% пациентов) и безопасно.
Практические рекомендации.
1. Больные, страдающие хроническим, длительно текущим, рецидивирующим циститом, не подающимся эффективному лечению с помощью стандартной фармакотерапии, должны подвергаться комплексному урологическому обследованию. Такое обследование обязательно должно включать в себя цистоскопию с биопсией мочевого пузыря, а также морфологическое и имуногистохимическое изучение полученных микропрепаратов.
2. У пациенток с морфологическими признаками интерстициального цистита — дефицитом гликозаминогликанов, — независимо от нозологической формы хронического цистита, возраста, наличия либо отсутствия данных за наличие инфекции мочевых путей, может быть эффективным лечение гиалуронатом цинка внутрипузырно.
3. Пациенткам в постменопаузе, без противопоказаний для проведения локальной заместительной гормональной терапии, показана местная гормонозаместительная терапия эстриолом.
4. Внутрипузырное введение раствора гиалуроната цинка эффективно и безопасно при схеме введения по 20 миллилитров (10 миллилитров гиалуроната цинка с 10 миллилитрами физиологического раствора) в опорожненный мочевой пузырь, один раз в три дня, курс лечения 10 инсталляций.
5. Контрольная цистоскопия с биопсией стенки мочевого пузыря после лечения гиалуронатом цинка должна выполняться не ранее, чем, через 21 день после завешения лечения.
