Связь изменений перфузии миокарда у больных коронарной болезнью сердца с выраженностью системной воспалительной реакции и гипергомоцистеинемией (реферат, курсовая, диплом, контрольная)
Актуальность проблемы. Несмотря на проводимые в последние годы профилактические мероприятия, в России продолжает неуклонно расти смертность от сердечно-сосудистых заболеваний. Так, по мнению экспертов, этот показатель в РФ за последние 12 лет возрос на 35% [Ю.А.Карпов, Е. В. Сорокин, 2002].
Тщательный анализ причин роста смертности от сердечно-сосудистых заболеваний позволил сделать вывод о том, что такие «традиционные» факторы риска, как гипертония, нарушения липидного обмена, курение не оказывают существенного влияния на этот процесс [Чазов Е.И., 2004].
В последнее время значительно возрос интерес исследователей к так называемым «нетрадиционным» или «нелипидным» факторам риска атеросклероза и коронарной болезни сердца (КБС) [Кашежева А.З., Ефимов B.C., 2000; Шмелева В. М, 2000; Сидоренко Г. И., Мойсенок А. Г., Колядко М. Г. и др., 2001; Баркаган З. С., Костюченко Г. И., Котовщикова Е. Ф., 2002; Павликова Е. П., Мерай И. А., 2003; Костюченко Г. И., Баркаган З. С., 2004; Чазов Е. И., 2004; Lewandowski К., 1998; Harjai К.J., 1999; Malinow M.R., Bostom A.G., Krauss R. M, 1999].
Интенсивно изучаются в этом аспекте гипергомоцистеинемия (ГГЦ), а также маркеры воспаления — С-реактивный белок (СРБ), интерлейкин-6 (ИЛ-6), фибриноген (Фн), ингибитор тканевого активатора плазминогена (ИТАП), а также некоторые эндотелиальные факторы, из которых наиболее важными являются — эндотелин-1 (ЭТ-1) и оксид азота (NO) [Ольбинская Л.И., Игнатенко С. Б., 2001; Алексеева И. А., Лякишев А. А., Ткачук В. А., и др., 2002; Арефьева Т. И., Проваторов С. И., Самко А. Н. и др., 2002; Meade T.W., Rudock V., Stirling Y., et al., 1993; Heinrich J., Balleisen L., Schulte H., et al., 1994; Koenig W., Hoffmeister H.C.A., Nehmer C., et al., 1994; Montalescot G., Ancri A., Vicaunt E., et al., 1995].
Среди перечисленных «нелипидных» факторов риска и маркеров особое место занимают ГГЦ и маркеры воспаления.
Об этом говорит значительный рост числа исследований, посвященных изучению роли ГГЦ и воспаления в патогенезе сосудистых заболеваний. Так, если в 1995 г. по проблеме ГГЦ было опубликовано примерно 200 работ, то в 2002 году — более 850, в том числе 3 монографии [Bertsch Т., Miellce О., Haly S. et all., 2001; Jong S., 2001; Stehouwer C.D., 2001 и другие].
На последних Европейских и Американских конференциях, посвященных проблемам атеросклероза, воспалительная теория явно преобладала [Gallagher РМ., 2004].
В результате проведения более 100 контролируемых исследований, выполненных в последние годы, показано, что даже умеренная ГГЦ способствует развитию коронарной недостаточности и высокой смертности. ГГЦ приводит не только более чем 3-кратному увеличению риска КБС, но и является важным критерием неблагоприятного ее клинического течения [Nygard О., 2000]. При повышении уровня ГЦ крови на 3 мкмоль/л смертность от сердечнососудистых заболеваний у таких больных возрастает на 20% [Nygard О., Nordrehaug JE., Refsum Н., et all., 1997; Wald N.J., Watt НС., Law MR., et all., 1998]. Повышение концентрации ГЦ в крови более 20 мкмоль/л приводит к увеличению смертности более, чем в 4 раза [Gallagher РМ., Meleady R., Shields D.C., et all., 1996]. Установлено, что общая смертность и смертность от сердечнососудистых заболеваний находятся в прямой зависимости от уровня ГЦ в крови, причем вне зависимости от наличия или отстутствия у пациентов других традиционных факторов риска [Malinow M.R., 1990, 1994; Graham I.M., Daly L.E., Refsum H.M., et all., 1997].
Помимо ГГЦ в последние годы все большее значение в патогенезе КБС отводится воспалению [Карпов Ю.А., Сорокин Е. В., 2002; Костюченко Г. И., 2004; Rudoclc V., Stirling Y., et al., 2003; Heinrich J., Balleisen L., Schulte H., et al., 2004]. Общепринято, что воспаление имеет наибольшее значение в дестабилизации атеросклеротической бляшки (нарушении целостности ее покрышки с последующим тромбозом места разрыва), являющейся морфологическим субстратом острых коронарных синдромов.
Показано, что у 50−70% больных нестабильной стенокардией класса III В по Е. Braunwald отмечается повышение содержания СРБ в плазме крови. Стойко увеличенные концентрации СРБ у больных с успешно купированным обострением ИБС считают предвестником повторной дестабилизации атеросклеротической бляшки и рецидива острого коронарного синдрома [Vicaunt E., etal., 1995].
По данным L. Biasucci и соавт. (2001), у больных нестабильной стенокардией с концентрацией СРБ, измеренной при поступлении, выше 3 мг/л риск развития острого инфаркта миокарда и смерти в стационаре был существенно выше. В исследовании FRISC (Fragmin during instability in Coronary artery disease) у больных нестабильной стенокардией, рандомизированных для лечения дальтепарином (фрагмином), в течение 3-летнего наблюдения высокий риск смерти (7,5%) был отмечен в группе с большими концентрациями СРБ (>10 мг/дл), но вероятность неблагоприятного исхода была повышена (3,6%) и при более низком его содержании в плазме крови (2−10 мг/дл). СРБ наряду с тропонинами был независимым предиктором сердечно-сосудистой смерти.
Суммируя вышеизложенное молено сказать, что в настоящее время ведущая роль в патогенезе атеросклероза и КБС отводится воспалительной реакции и ГГЦ.
В своей работе мы попытались изучить возможную связь между выраженностью ГГЦ, уровнем основного маркера воспаления (СРБ) и степенью нарушения перфузии миокарда у больных КБС, а также оценить влияние на изучаемые показатели специального витаминного комплекса, содержащего фолиевую кислоту, витамины В6 и В12.
Цель исследования. Оценить выраженность нарушений перфузии миокарда у больных КБС в зависимости от исходного уровня ГЦ и СРБ в крови, а также оценить влияние на эти показатели витаминного комплекса «Ангиовит», сотстоящего из фолиевой кислоты, витаминов Вб и Bi2.
Основные задачи исследования.
1. Изучить выраженность нарушений перфузии миокарда у больных КБС с различным исходным уровнем гомоцистеина в крови.
2. Определить возможную связь между степенью нарушения перфузии миокарда и выраженностью воспалительной реакции, контролируемой по уровню С-реактивного белка в крови, у больных КБС.
3. Изучить влияние гомоцистеинкоррегирующей терапии на выраженность нарушений перфузии миокарда у больных КБС.
4. Изучить влияние гомоцистеинкоррегирующей терапии на концентрацию СРБ в сыворотке крови больных КБС.
Научная новизна.
Впервые в рандомизированном контролируемом исследовании была доказана связь между уровнем СРБ в крови и выраженностью нарушений перфузии миокарда у больных КБС. Показана возможность положительного влияния на патологически измененную перфузию миокарда у больных КБС посредством включения в комплекс терапевтических средств специального витаминного комплекса «Ангиовит». Установлено, что прием в течение двух месяцев витаминного комплекса «Ангиовит» приводит не только к снижению в крови уровня ГЦ, но также и концентрации СРБ — основного маркера воспалительной реакции.
Практическая значимость работы.
Включение в комплекс терапевтических средств, применямых у больных КБС витаминного комплекса «Ангиовит» позволяет с высокой эффективностью и в сравнительно короткие сроки, без каких — либо серьезных побочных эффектов, добиться улучшения перфузии миокарда у большинства больных КБС. Кроме того, установлено, что прием витаминного комплекса «Ангиовит» способствует снижению уровня СРБ в крови, что по нашему мнению, существенно расширяет терапевтические возможности этого препарата. Основные положения, выносимые на защиту.
1. Степень нарушения перфузии миокарда у больных КБС находится в прямой связи с уровнями ГЦ и СРБ в крови.
2. Коррекция уровня ГЦ в крови с помощью витаминного комплекса способствует улучшению перфузии миокарда у 92% больных КБС.
2. Включение витаминного препарата «Ангиовит» в комплекс терапевтических средств при КБС позволяет снизить у большинства пациентов не только уровень ГЦ, но и концентрацию СРБ в крови.
Объем и структура диссертации.
Диссертация состоит из введения, трех глав (посвященных аналитическому обзору литературы, описанию материалов и методик исследования, результатам собственных исследований), заключения, выводов, практических рекомендаций. Библиографический указатель литературы включает 230 источников, из них 23 отечественных и 207 — иностранных авторов. Объем работы — 113 страниц, она иллюстрирована 17 таблицами, 4 диаграммами, 1 рисунком.
выводы.
1. У больных коронарной болезнью сердца выявлена прямая связь между степенью нарушения перфузии миокарда и исходными уровнями гомоцистеина и С-реактивного белка в крови (г=+0,47- р<0,05).
2. Коррекция уровня гомоцистеина в крови больных коронарной болезнью сердца при помощи витаминного комплекса «Ангиовит» сопровождается достоверным улучшением перфузии миокарда у 92% пациентов (р<0,02).
3. Гомоцистеинкоррегирующая терапия способствует уменьшению выраженности системной воспалительной реакции у больных коронарной болезнью сердца, что подтверждается снижением концентрации С-реактивного белка в крови (р<0,05).
ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ.
Как показали наши исследования включение препарата «Ангиовит» в комплекс терапевтических средств, применяемых для лечения больных коронарной болезнью сердца, позволяет не только снизить влияние таких факторов риска развития осложнений заболевания, как гипергомоцистеинемия и воспалительная реакция, но и существенно улучшить перфузию миокарда и клиническое течение заболевания. Больным с установленным диагнозом коронарной болезни сердца рекомендуется определение гомоцистеина в сыворотке крови. При уровне гомоцистеина превышающем 11 мкмоль/л показано включение в схему лечения гомоцистеинкоррегирующего фолатного комплекса «Ангиовит», содержащего фолиевую кислоту (5мг), витамин Вб (4 мг), витамин В и (6 мкг).
Препарат назначается по 1 таблетке два раза в день, вне зависимости от приема пищи. Суточная доза препарата в коррекции на степень превышения контрольного показателя ГЦ не нуждается. 3 По окончании 2-х месячного курса лечения, пациенту назначается поддерживающая терапия в дозе 1 таблетка в день [Г.И. Костюченко, З. С. Баркаган, 2004]. Учитывая комбинацию фолиевой кислоты с витаминами Вб и В]2, лечение может продолжаться неопределенно долго. Уровень гомоцистеина крови контролируется ^ ежемесячно. При отсутствии эффекта возможен возврат в 2-х f месячному курсу в полной дозе.
Побочных эффектов витаминотерапии в нашем исследовании не зарегистрировано. Противопоказанием к назначению витаминного комплекса может явиться индивидуальная непереносимость, или аллергическая реакция на витамины группы В.
