ОБСУЖДЕНИЕ РЕЗУЛЬТАТОВ.
ВЫВОДЫ.
Функциональное состояние клеток крови и эндотелия у больных гипертонической болезнью, связь с ремоделированием сердца и крупных сосудов (реферат, курсовая, диплом, контрольная)
АКТУАЛЬНОСТЬ ПРОБЛЕМЫ Структурные и функциональные изменения, возникающие в сердце и сосудах при гипертонической болезни (ГБ), являются самостоятельной причиной дальнейшего прогрессирования заболевания и независимым негативным прогностическим фактором (Devereux R. et al., 2000).
Так, наличие гипертрофии левого желудочка (ГЛЖ) сопряжено с более высоким риском сердечно-сосудистой патологии, в том числе инфаркта миокарда, инсульта, сердечной недостаточности, желудочковых нарушений ритма и внезапной смерти (Levy D. et al., 1988; Kannel W.B., 1991; Koren M.J. et al., 1991; Bikkina M. et al, 1994; Levy D. et al., 1996; Devereux R.B., Roman M.J., 1999). По данным проспективных исследований толщина комплекса интима-медиа (КИМ) общих сонных артерий является предиктором сердечно-сосудистой заболеваемости и смертности (Chambless L.E. et al., 1997; Bots M.L. et al., 1999; O’Leaiy D.H., 1999; Greenland P. et al., 2000; Touboul P.-J. et al., 2000). Недавно было показано, что наличие эндотелиальной дисфункции, по данным оценки степени вазодилатации при реактивной гиперемии и определения повышенного содержания фактора Виллебранда (фВ), обладает прогностическим значением у больных ГБ в отношении развития сердечно-сосудистых осложнений (Рег-ticone F. et al., 2001; Spencer C.G.C. et al., 2002).
Несмотря на то, что при ГБ кровеносные сосуды подвергаются воздействию высокого артериального давления (АД), основные осложнения — инфаркт миокарда и инсульт — носят тромботический, а не геморрагический характер (Lip G.Y.H., 2000). При ГБ имеет место полная триада Вирхова, лежащая в основе тромбообразования: изменения сосудистой стенки (дисфункция эндотелия), кровотока (нарушение реологических свойств крови, изменения физических свойств потока, снижение напряжения сдвига) и состава крови (активация свертывающей системы, нарушение фибрино-лиза, повышение адгезии и агрегации тромбоцитов). В литературе отсутствует четкое представление о влиянии этих факторов на процессы ремоделирования сердечнососудистой системы при артериальной гипертензии (АГ).
В настоящее время активно обсуждается роль воспаления в патогенезе АГ, ассоциированных с ней поражений органов-«мишеней» и их дисфункции. Прежде всего, это связано с участием иммунных механизмов в ремоделировании артерий и ускорении процессов атерогенеза при AT (Sullivan G.W. et al., 2000). Активация лейкоцитов может быть ассоциирована с увеличением продукции свободных кислородных радикалов, провоспалительных цитои хемокинов, участвующих в ремоделировании сосудистой стенки и миокарда (Dorffel Y. et al., 2001). Однако функциональная активность лейкоцитов у больных ГБ практически не изучена.
Известно, что, несмотря на достижение целевого уровня АД в процессе антиги-пертензивной терапии, не всегда удается улучшить прогноз, снизить риск развития сердечно-сосудистых осложнений. Возможно, одной из причин является отсутствие благотворного влияния антигипертензивных препаратов на состояние клеток крови и эндотелия. Поэтому анализ динамики функционального состояния клеток крови и эндотелия на фоне ан^игипертензивной терапии представляется актуальным.
Проблемам связи функциональной активности клеток крови и эндотелия с процессами сердечно-сосудистого ремоделирования у больных гипертонической болезнью и ее изменениям в процессе терапии посвящено настоящее исследование.
ЦЕЛЬ ИССЛЕДОВАНИЯ.
Оценить реологические свойства крови, функциональную активность лейкоцитов и эндотелия у больных гипертонической болезнью, их роль в ремоделировании сердца и крупных сосудов, динамику на фоне антигипертензивной терапии.
ЗАДАЧИ ИССЛЕДОВАНИЯ.
1. Исследовать функциональную активность клеток крови у больных гипертонической болезнью.
2. Изучить связь между структурно-функциональным состоянием сердца и изменениями активности клеток крови у больных гипертонической болезнью.
3. Охарактеризовать связь между структурными изменениями крупных артерий и функциональной активностью клеток крови и эндотелия у больных гипертонической болезнью.
4. Оценить динамику состояния клеток крови и эндотелия, ремоделирования сердца и крупных сосудов у больных гипертонической болезнью на фоне длительной терапии ингибиторами ангиотензин-превращающего фермента и бета-адреноблокаторами.
ПОЛОЖЕНИЯ, ВЫНОСИМЫЕ НА ЗАЩИТУ.
Нарушения функционального состояния клеток крови, наряду с клиническими и гемодинамическими детерминантами, являются дополнительными факторами ремо-делирования сердца и крупных сосудов при гипертонической болезни.
На фоне длительной антигипертензивной терапии ингибитором ангиотензин-превращающего фермента эналаприлом, в отличие от терапии бета-адреноблокатором атенололом, независимо от выраженности гипотензивного эффекта препарата улучшаются реологические свойства крови, снижается функциональная активность лейкоцитов и проагрегантная способность эндотелия.
НАУЧНАЯ НОВИЗНА.
Продемонстрировано повышение функциональной активности лейкоцитов с экспрессией ранних маркеров активации и молекул адгезии на лимфоцитах у больных гипертонической болезнью.
Установлено, что изменения реологических свойств крови сопряжены с активацией лейкоцитов при гипертонической болезни.
Обнаружено, что повышение функциональной активности лейкоцитов ассоциировано со снижением эндотелийзависимой вазодилатации лучевой артерии в пробе с реактивной гиперемией и повышением концентрации фактора Виллебранда в плазме крови, характеризующими эндотелиальную дисфункцию у больных гипертонической болезнью.
Установлено, что развитие концентрического типа гипертрофии левого желудочка у больных гипертонической болезнью сопровождается более выраженными реологическими нарушениями, увеличением функциональной активности лейкоцитов и эндотелия.
Выявлено, что увеличение толщины комплекса интима-медиа общих сонных артерий связано с активацией лейкоцитов у больных гипертонической болезнью.
Показано, что терапия ингибитором ангиотензин-превращающего фермента эналаприлом сопровождается снижением функциональной активности лейкоцитов у больных гипертонической болезнью.
ПРАКТИЧЕСКАЯ ЗНАЧИМОСТЬ Исследование реологических свойств крови, активности лейкоцитов, концентрации фактора Виллебранда в плазме крови позволяет выделить группу больных гипертонической болезнью с повышенным риском развития сердечнососудистых осложнений, связанных с процессами тромбои атерогенеза. Показано, что нарушения реологических свойств крови, активация лейкоцитов, повышение концентрации фактора Виллебранда в плазме крови ассоциированы с выраженностью структурно-функциональных изменений сердца и крупных сосудов у больных гипертонической болезнью.
Длительная антигипертензивная терапия эналаприлом сопровождается снижением концентрации фактора Виллебранда в плазме крови, агрегации эритроцитов и тромбоцитов, экспрессии ранних маркеров активации и адгезионных молекул на лимфоцитах независимо от условия достижения целевого уровня артериального давления. На фоне терапии атенололом у больных гипертонической болезнью функциональное состояние эндотелия, реологические свойства крови не улучшаются, а также не снижается активность лейкоцитов.
ВНЕДРЕНИЕ РЕЗУЛЬТАТОВ ИССЛЕДОВАНИЯ Результаты исследования внедрены в практическую работу и учебный процесс в Научно-исследовательском институте кардиологии имени В. А. Алмазова Министерства здравоохранения и социального развития РФ (Санкт-Петербург), в практическую работу кардиологического отделения городской больницы № 2 (Санкт-Петербург).
АПРОБАЦИЯ РЕЗУЛЬТАТОВ ИССЛЕДОВАНИЯ Материалы выполненного исследования представлены в виде докладов на Российском национальном конгрессе кардиологов (Санкт-Петербург, 2002), конгрессе кардиологов стран СНГ (Санкт-Петербург, 2003), Российском национальном конгрессе «Человек и лекарство» (Москва, 2003), Европейском конгрессе кардиологов (Вена, 2003), научно-практической конференции в рамках медико-социальной акции «Неделя здорового сердца и мозга» (Санкт-Петербург, 2002, 2003, 2004), заседаниях проблемной комиссии НИИ кардиологии имени В. А. Алмазова (Санкт-Петербург, 2003, 2004). По результатам диссертации опубликовано 13 работ.
СТРУКТУРА И ОБЪЕМ ДИССЕРТАЦИИ
Диссертация изложена на 162 страницах машинописного текста, состоит из введения, 4 глав, обсуждения и выводов. Указатель литературы включает 338 источников, из них 24 отечественных и 314 иностранных. Работа содержит 16 рисунков и 32 таблицы.
выводы.
1. У больных гипертонической болезнью II стадии повышена агрегация эритроцитов и тромбоцитов, снижена деформируемость эритроцитов, увеличена экспрессия ранних маркеров активации лимфоцитов и адгезионных рецепторов Мас-1 на лейкоцитах.
2. Изменения реологических свойств крови и активности лейкоцитов у больных гипертонической болезнью II стадии сопряжены с развитием эндотелиальной дисфункции, проявляющейся недостаточной вазодилатацией лучевой артерии в пробе с реактивной гиперемией и увеличением концентрации фактора Виллебранда в плазме крови.
3. У больных гипертонической болезнью процессы ремоделирования сердца и крупных артерий имеют общие демографические, гемодинамические детерминанты, а также связаны с изменениями реологических свойств крови и активацией лейкоцитов.
4. Нарушение функционального состояния эндотелия у больных гипертонической болезнью ассоциировано со структурными изменениями сердца и крупных артерий.
5. Длительная терапия атенололом не сопровождается улучшением реологических свойств крови, снижением функциональной активности лейкоцитов и эндотелия при адекватном гипотензивном эффекте препарата.
6. На фоне антигипертензивной терапии эналаприлом улучшается функциональное состояние эндотелия, проявляющееся снижением концентрации фактора Виллебранда в плазме крови, уменьшается агрегация эритроцитов и тромбоцитов, экспрессия ранних маркеров активации и адгезионных рецепторов Мас-1 на лимфоцитах.
ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ.
Выявление повышенной агрегационной способности эритроцитов и тромбоцитов, деформируемости эритроцитов, экспрессии поздних маркеров активации лимфоцитов и адгезионных молекул Мас-1 на лейкоцитах, концентрации фактора Виллеб-ранда в плазме крови у больных гипертонической болезнью позволяет выделить пациентов с более высоким риском развития ремоделирования сердца.
У больных гипертонической болезнью II стадии с повышенной агрегацией эритроцитов и тромбоцитов, экспрессией ранних маркеров активации и адгезионных молекул на лимфоцитах целесообразно использовать в качестве антигипертензивной терапии ингибитор ангиотензин-превращающего фермента эналаприл, так как его применение сопровождается улучшением реологических свойств крови, снижением функциональной активности лейкоцитов вне зависимости от выраженности гипотензивного эффекта препарата.
Больным гипертонической болезнью с нарушенной функцией эндотелия показана терапия инигибитором ангиотензин-превращающего фермента эналаприлом, так как на фоне его применения снижается концентрация плазменных маркеров повреждения эндотелия.
Лечение больных гипертонической болезнью бета-адреноблокатором атенололом менее желательно, так как не приводит к снижению активности лейкоцитов, улучшению реологических свойств крови и функционального состояния эндотелия.
