Эффективность иммуносупрессивной терапии у детей с рефрактерной апластической анемией (реферат, курсовая, диплом, контрольная)
Актуальность работы. Апластическая анемия (АА) у детей — относительно редкая, но чрезвычайно тяжелая патология. До внедрения современных методов лечения — трансплантации костного мозга и комплексной имму-носупрессивной терапии (ИСТ) — это заболевание неминуемо приводило к смерти больного, часто в короткие сроки [Camitta В.М. et al., 1976, 1979; Young N. et al., 1995, 1997; Heimpel H., 2000, Масчан AA, 2004].
Природа AA в значительном числе случаев остается неизвестной (идиопатическая АА), хотя у части больных ее удается установить (постгепатитная, «левомицетиновая» и др.) [Camitta В. et al., 1974, 1979, 1982; Kaufman D. et al., 1991; Young N., 1990, 1999].
Диагноз AA ставится на основании клинической картины, исследования крови и костного мозга [Camitta В. et al., 1975, 1982; Maciejewski J. et al., 1997; Scopes J. etal., 1994].
Основные принципы лечения апластической анемии, которых придерживаются врачи и исследователи в разных странах мира, мало отличаются. Считается, что эффективность лечения зависит от ряда обстоятельств: длительности периода от начала заболевания до постановки диагноза и начала адекватной терапии, тяжести заболевания, тактики иммуносупрессивной терапии и т. д. [Camitta В, Doney К., 1990; Young N.S., Barrett A.J., 1995; Passweg J. et al., 1997; Bacigalupo A. et al., 1995, 2000; Killick S.B., Marsh J.C.W., 2000].
Очень важно на ранних этапах лечения получить представление о том, каков прогноз заболевания. Однако, работ, посвященных этой проблеме, очень мало и они основаны на оценке лишь отдельных параметров. Еще меньше работ посвящено оценке прогноза течения заболевания и ответа на лечение у пациентов, терапия первой линии у которых не привела к достижению гематологической ремиссии, что встречается в 30−50% случаев. Отмечается явная недостаточность крупных исследований, посвященных терапии рефрактерных случаев заболевания. В связи с вышеизложенным, актуальным является проведение комплексных исследований эффективности терапии у больных с резистентной АА для решения вопроса о том, какие клинические и лабораторные признаки могут служить критериями вероятности «ответа» на повторную ИСТ. Это могло бы быть важным при принятии решения о целесообразности риска резкой эскалации терапии, например, проведения трансплантации стволовых клеток от альтернативного донора или использования иммуносупрессивных препаратов «второй линии» [Deeg HJ. et al., 1999; Di Bona E. et al., 1999; Schrezenmeier H., Bacigalupo A., 2000; Bacigalupo A. et al., 2000; Killick S.B., Marsh J.C.W., 2000; Масчан А. А. и соавт., 2001; Brodsky R.A. et al., 2004].
Цель работы. Определить значение инициальных клинико-лабораторных показателей больных с АА, не ответивших на первый курс ИСТ в течение 6 месяцев и их динамики в процессе проведения первого и последующих курсов ИСТ в определении прогноза и улучшения окончательных результатов лечения.
Задачи работы.
1. Охарактеризовать инициальные (до лечения) клинико-лабораторные показатели у детей, больных АА, рефрактерной к первому курсу ИСТ и определить их значение в прогнозе окончательного ответа;
2. Определить влияние различных комбинаций препаратов в 1-м и последующих курсах ИСТ на окончательные результаты терапии;
3. Охарактеризовать изменения, происходящие в клинических и лабораторных показателях у детей с рефрактерной АА, в процессе проведения курсов ИСТ и определить их прогностическое значение;
4. Оценить частоту и характеристики минимального ответа на курсы ИСТ и определить их прогностическое значение;
5. На основании выявленных прогностических факторов, создать адекватный терапевтический алгоритм для пациентов, не ответивших на первый курс ИСТ.
Научная новизна работы. Впервые в отечественной педиатрической гематологии проанализированы результаты ИСТ большой группы пациентов (56) с рефрактерной АА и показано, что при условии настойчивого проведения ИСТ на фоне современной сопроводительной терапии, возможно добиться ответа при проведении повторных курсов ИСТ у 43% пациентов и достичь 49% 8-летней общей выживаемости. Установлено, что у детей, больных АА, рефрактерной к первому курсу ИСТ, имеются существенные отличия в ряде клинико-лабораторных показателей в течение и по окончанию первых двух курсов ИСТ («ранний» гранулоцитарный ответ и максимальное количество нейтрофилов в течение всего курса, минимальный промежуточный ответминимальный окончательный ответчастота эпизодов инфекции, требующих применения парентеральных антибактериальных и противогрибковых препаратов), зависящие от того, удалось или не удалось в итоге добиться ответа на ИСТ. Данные показатели могут использоваться в качестве прогностических при планировании терапии у пациентов с рефрактерными АА.
Практическое значение работы. Показано, что интенсивность ИСТ у пациентов с АА не должна снижаться при отсутствии ответа на первый курс лечения. Установлено, что у детей, больных рефрактерной АА, определение ряда параметров в течение и по окончанию курсов ИСТ («ранний» гранулоцитарный ответ и максимальное количество нейтрофилов в течение курса ИСТ, характер минимального промежуточного и окончательного ответовчастота эпизодов инфекции, требующих применения парентеральных антибактериальных и противогрибковых препаратов) имеет прогностическое значение и может использоваться для определения оптимальной тактики терапии. Обоснована терапевтическая тактика у больных с рефрактерной АА в зависимости от характеристик ответа на 1-й и 2-й курсы ИСТ. Разработан и внедрен в практику алгоритм ведения детей с рефрактерной АА.
Положения, которые выносятся на защиту:
1. Влияние инициального клинико-лабораторного статуса и особенностей первых курсов иммуносупрессивной терапии (сочетания использованных лекарственных препаратов) на окончательные результаты лечения.
2. Взаимосвязь клинико-лабораторных изменений, произошедших в процессе проведения курсов ИСТ и окончательных результатов лечения.
3. Влияние характеристик минимального ответа, достигнутого в ходе ИСТ на окончательные результаты лечения.
Внедрение результатов исследования в практику. Полученные теоретические и практические результаты работы используются в лекционном курсе для клинических ординаторов и курсантов ФУВ на кафедре гематологии/онкологии и иммунопатологии с курсом поликлинической и социальной педиатрии РГМУ, а также в практической работе отделений гематологии РДКБ Росздрава и МДГКБ г. Москвы.
Публикации. По теме диссертации опубликовано 2 печатные работы.
Апробация работы. Основные положения работы обсуждались в коллективах отделения общей гематологии РДКБ Росздрава и отделов НИИ детской гематологии Росздрава. Апробация диссертации проведена на научно-практической конференции НИИ детской гематологии, совместно с сотрудниками РДКБ г. Москвы 30 мая 2005 г.
Структура и объем работы. Диссертация изложена на 190 страницах машинописного текста и состоит из: введения, обзора литературы, 4 глав собственных исследований, заключения, выводов, практических рекомендаций. В тексте содержится 78 таблиц и 24 рисунка. Библиография включает 257 источников литературы, из них 26 на русском и 231 на иностранных языках.
Выводы.
1. Рефрактерная АА у детей, несмотря на интенсивную ИСТ, остается очень тяжелым заболеванием, в 45% случаев приводящим к смерти. Только у 35% больных удается достичь длительных полных и парциальных ремиссий. Общая 8-летняя выживаемость составляет 49%.
2. У детей, больных АА, ответивших и не ответивших на ИСТ, до начала лечения не обнаружено различий в большинстве инициальных клинических и лабораторных параметрах. Те же показатели, по которым обнаружены достоверные различия, не могут быть использованы в качестве прогностических.
3. Не обнаружено различий в окончательных результатах лечения в зависимости от комбинации препаратов, использованных в процессе проведения 1 и 2 курсов ИСТ.
4. Оценка изменений в клинических и лабораторных проявлениях заболевания в процессе проведения ИСТ является основным критерием ближайших и отдаленных результатов лечения. У больных, ответивших на ИСТ, достоверно чаще имел место выраженный гранулоцитарный ответ и было выше максимальное количество нейтрофилов в течение первых 3-х недель первых 2-х курсов ИСТ, реже отсутствовал минимальный промежуточный ответ.
5. Минимальный «окончательный» ответ после курсов ИСТ достоверно чаще отсутствовал у больных, не ответивших в итоге на терапию. В процессе развития минимального ответа у больных, ответивших на ИСТ, достоверно чаще имели место сочетанные изменения, его характеризующие. Изолированное снижение трансфузионной потребности в препаратах крови не являлось положительным прогностическим признаком ответа на терапию.
6. У больных, ответивших в итоге на ИСТ, в процессе проведения первых 2-х курсов терапии частота эпизодов инфекций была достоверно меньше, что снизило объем назначений парентеральных антибактериальных и противогрибковых препаратов.
7. Пациенты, никак не отвечающие на иммуносупрессивную терапию после 1-го и, особенно, 2-го курсов ИСТ, имеют крайне малую вероятность ответа на последующие курсы терапии и низкую выживаемость, что требует максимально скорого проведения у них трансплантации гемопоэти-ческих стволовых клеток от альтернативных доноров.
Практические рекомендации.
1. У детей, больных апластической анемией, рекомендуется определение ряда параметров в течение и по окончанию первых курсов (1 и 2) иммуносупрессивной терапии:
• «быстрый» гранулоцитарный ответ и максимальное количество нейтрофилов в течение первых 3-х недель;
• минимальный промежуточный ответ;
• минимальный окончательный ответ, в первую очередь снижение тяжести апластической анемии и частоты трансфузий компонентов крови;
• частота эпизодов инфекции, требующих применения антибактериальных и противогрибковых препаратов.
Это необходимо, поскольку определение указанных параметров имеет прогностическое значение — так как эти показатели существенно (достоверно) отличаются у больных ответивших и не ответивших (в итоге) на иммуносупрессивную терапию.
2. Пациенты, у которых в ходе проведения первого курса комбинированной ИСТ отмечен только минимальный ответ, должны не позднее 6 месяцев от его начала получить повторный курс терапии.
3. Пациенты, ответившие на повторный курс комбинированной ИСТ только минимально, должны рассматриваться как потенциальные кандидаты на проведение аллогенной ТКМ, в т. ч. от альтернативного донора, однако, им может быть проведен третий курс ИСТ в надежде на поздний гематологический ответ.
4. Пациенты, не ответившие на 2 курса комбинированной ИСТ с включением АТГ и ЦСА даже минимальным улучшением, должны быть, при возможности, подвергнуты аллогенной ТКМ от альтернативного донора в максимально возможные сжатые сроки.
Независимо от наличия или отсутствия ответа на ИСТ и возможности проведения аллогенной ТКМ, прекращение ИСТ или снижение ее интенсивности не должно допускаться.
